This is a pretty good explanation of it. In some cases, it is used as an "other" offense when somebody has done something really awful that isn't properly covered by our other offense reasons.
Bans and such aren't public. You generally won't know when or what actions are taken against a certain person.
I'm finding it hard to understand what you're trying to say. Some of those won't result in a ban at all, unless you're still doing it after being disconnected and warped a few times.
If your question is if it is used because you've been banned for a lot of different things, the answer is no.

What does "false ban" means? Some people usually get banned for a long time with multiple bans (apparently, for no reason) then they send an appeal ticket and admins check if it's a false ban, if it was they un-ban the person.
Does it mean when admins accidentally ban people?
