Do they change when you change your look? Like, say you had an NPC somewhere of yourself, and then you changed your look, does it update to the new one?

Nope, I've seen some people's NPC's from a year ago and their look/name/info differs majorly.

The head or body will be changed to head0.png and body0.png if you refund it.
